VoIP software is complex and has a large attack surface. Several threat classes have been identified in the literature: social threats; eavesdropping, interception and modification of session contents; denial of service attacks; unauthorized access to VoIP equipment; and a sudden interruption of services.

Anonymizing VoIP is somewhat difficult, but still possible. It is easy to hide the IP address with Whonix, but the voice recognition component and slow Tor network speed (latency) are definite obstacles. Depending on your threat model, pseudonymous use of VoIP might be appropriate; this involves hidden voice communications that take place with known contacts.
